What if I can't get PDF forms?

Some pages attached to our site are in PDF format.  To reach these pages, you need Adobe Acrobat Reader.  If you do not have this program, when you click on your mouse to view the form, you won't be able to see it.

You can get the Reader for free from the Adobe site. To download it, click the icon below and follow the instructions. (At Step #2, remember to look at the check boxes. If you have an older computer without much memory, you may want not want the "full version". You may also want to uncheck other boxes with offers of demo software or other information.)

If for some reason you can't make this work, or don't want the Reader, call us.  We will mail you the forms. For court forms, go to your nearest district court.

How do I get interactive PDF forms to work?

When you open the page, using your mouse, put your cursor at the beginning of the first line.  Type in the appropriate information.  Then hit the tab key to go to the next blank.

To check a box, using the mouse, place your cursor on the box. After the picture with the pointing finger appears, click on the box.  If you made a mistake, click again and the check mark will go away.  To go on to the next box or empty line, hit the tab key.

When you're all done, print.

I have Acrobat Reader but I can't get the self-calculating forms to work. Why not?

A few of our forms will do math calculations for you.  However, you need a recent version of Adobe Reader for them to work well. We recommend using the most recent version or, at least 7.0. For more help take a look at our Troubleshooting Page.

How can I save my forms using CutePDF or Foxit Reader?

To save your completed forms, the best software we've found is either CutePDF's Form Filler or Foxit Reader Click to get Foxit Reader. These are both easy-to-use products. Foxit is free, and CutePDF inexpensive. When you go to their site, you will see that they are offering a free evaluation version. If you try it and like it, and you do a lot work with .pdf forms, you can buy this software for only $29.95.

Here is one way to get the software and use it:

  • Step 1: Go to one of the above websites and follow the instructions for downloading the software onto your computer.
  • Step 2: Find the form you want and "right click" to download it to your computer
  • Step 3: Save the file to your desktop or a folder where you can find it.
  • Step 4: Open Foxit Reader or CutePDF Form Filler and click on "open."
  • Step 5: Browse and select the file you saved on your computer to open it.
  • Step 6: Fill out the form and then save it as you would any file.

You'll notice that many forms include a "Reset" button. If you click on this, you will clear the form so that the next user will see a blank form, not your completed form.

How do I find a specific drug on the MaineCare Preferred Drug List?

Go to the Preferred Drug List site. If you have the software to view it, open the most recent Excel version. The list is very long. "Preferred Drugs" are listed in Column C (third column from the left). To find a drug quickly, use your mouse to click on any drug. Then hit Control-F on your keyboard--hitting F while holding down the Control key. Type the name of the drug you are looking for in the field labeled "Find what:" Then press the Enter key. You must be careful to spell the drug correctly. If your spelling doesn't match up, the program won't find your drug. If your drug's name has several words, you can type in just one of the words. (Example: A search for "monohydrate" will find "cephalexin monohydrate.") Notice that if your drug shows up in the fifth Column, Column E, it is a "non-preferred drug" and must get special approval. If you cannot view an Excel file on your computer but you have Adobe Acrobat Reader, you can veiw that version of the list. It will be harder to use because you can't search the document quickly, as you can with the Excel version.
